What exactly does Hallucination mean?
This is not a defect of individual models, but a consequence of how they work: they generate the most plausible continuation, not the one that is demonstrably correct. That is why hallucinations cannot be switched off, only contained.
The most effective countermeasures are three things in particular: binding the answer to your own documents (see RAG), making sources visible, and having a human make the decision for anything with legal consequences.
