AI phone assistant in home care: stay reachable while the team is out

In short

Outpatient care services are often hardly reachable during the day because care staff are out on their rounds. An AI phone assistant takes calls from relatives, new customers, and partners, sorts them by urgency, and forwards emergencies immediately.

The most important points at a glance

  • An AI phone assistant ensures that in outpatient care no call is lost, while the team is out on visits.
  • It handles new customer inquiries, messages from relatives and calls from medical practicesstructured.
  • Emergency keywords immediately trigger a reference to 112 and on-call availability.
  • Health data require data minimization, EU processing, AVV and deletion periods.
  • The introduction can be subsidized via INQA-Coaching with 80 % funding.

An AI phone assistant in home care services takes calls when the office is unattended or all caregivers are out on their rounds: It records inquiries from relatives and prospects, answers recurring questions and forwards urgent matters immediately to a reachable person. He does not make nursing or medical decisions – he ensures that no call is lost.

Why the phone becomes a bottleneck in home care services

In outpatient services, the nursing service manager is often also the route planner, contact person for insurers, and the person who steps in when staff are absent. Meanwhile, the phone keeps ringing: relatives ask about service times, prospective clients ask about available capacity, and doctors’ offices and hospitals ask for feedback. Anyone who cannot get through will call again at the next shift – especially with new client inquiries this leads to lost capacity.

Which calls an assistant can handle

  • New customer inquiries: Record care level, desired services, place of residence, and preferred callback time in a structured way – so the manager can call back with purpose.
  • Recurring questions: Service area, range of services, office hours, documents required for the initial visit.
  • Messages from relatives:Appointment requests, vacation absences, notes for the caregiver – written down and clearly assigned instead of on a note.
  • Calls from practices, clinics, and pharmacies:Record the request and pass it on to the responsible person.
  • Applications:Capture interested caregivers instead of sending them to voicemail.

The most important rule: urgent matters go to people immediately

An assistant in the care sector needs clear escalation rules. Keywords such as fall, shortness of breath, severe pain, or “unresponsive” do not lead to a note, but immediately to the instruction to call emergency services at 112 and to be forwarded to the on-call service. These rules are defined together with the nursing management and checked with test calls before launch.

Data protection: health data require special care

Information on care level, diagnoses, or medication are health dataand therefore particularly protected. This results in specific requirements:

  • Only collect the information that is truly necessary for the callback – data minimization.
  • Processing within the EU and a data processing agreement.
  • Clear deletion periods for call logs.
  • Clear notice that an AI assistant is speaking, and a way to reach a human.

How care facilities can use AI in compliance with data protection is also described in the article on Data protection and AI in care.

This is how implementation works

  1. Evaluate calls:For one week, note down who calls and why. This will reveal the tasks the assistant should take over.
  2. Set rules: What does it answer itself, what does it take a message for, what does it forward immediately?
  3. Start a pilot: initially only outside office hours or when the line is busy.
  4. Evaluate and expand:Review protocols, refine responses, expand deployment times.

You can find further use cases for care and social service providers on the industry page Care and social service providers, the technical solution on the page AI phone assistant. The Introduction of an AI phone assistant can be funded as a consulting project: through INQA-Coaching the federal government covers 80% of coaching costs nationwide. The funding applies to the support service, not the software itself. The funding check shows within a few minutes what is eligible for your business.

A typical day with and without an assistant

Without an assistant, it often goes like this: In the morning, management is busy with route planning and sick leave notifications; at midday, everyone is out on the road; in the afternoon, callback requests pile up on the answering machine – incomplete, hard to understand, without a callback number. In the evening, calls are returned to whoever is still reachable.

With an assistant, every call is answered and structured and stored: Who called, what for, how urgent, when is a callback possible? Management works through the list by priority instead of deciphering messages. New customer inquiries no longer get lost because nobody picked up.

Example dialogue: Inquiry from a family member

  • Assistant: "Good day, this is the care service’s digital assistant. How can I help?"
  • Caller: "My mother will need help with washing and with her tablets starting next month."
  • Assistant: asks for the place of residence within the service area, care level (if available), preferred times, and a callback number.
  • Result: structured new-customer inquiry in the manager’s inbox, confirmation to the caller that someone will get back to her.

The dialogue is illustrative; questions and wording are defined together with your team.

What the nursing service management should define

  • Which details are really needed for new customers — and which only in the initial consultation.
  • Who is on call and how they can be reached.
  • Which questions the assistant may answer itself (service area, services, office hours).
  • How long conversation logs are stored.

Experience from the social economy

We ourselves come from the social economy and have digitized processes there – for example for integration support at Caritas or in the AI strategy for the DRK Mecklenburg-Vorpommern. The key lesson: technology is accepted when the team helps shape it. That is why nursing staff and administration belong at the table from the very beginning – an approach that INQA-Coaching also requires.

What an AI phone assistant does not replace in nursing care

  • Personal conversations with relatives in stressful situations.
  • Professional nursing advice and the assessment of support needs.
  • Coordination with doctors about treatments.
  • The on-call duty itself – the assistant only forwards the call.

Clearly defined boundaries protect callers and the team alike. They belong in the greeting (“I will take your request and pass it on”) and in the assistant’s rules.

Checklist for getting started in home care services

  1. Call analysis over one to two weeks, separated by caller groups.
  2. Define and test emergency keywords and on-call availability.
  3. Clarify data protection: legal basis, data processing agreement, deletion periods, informing callers.
  4. Agree on the greeting message – friendly, brief, with a note about AI.
  5. Inform and involve the team, and define feedback channels.
  6. Pilot outside office hours, evaluate after four weeks.

Frequently asked questions

May an AI phone assistant in a care service collect health data?

Yes, if there is a legal basis, only the necessary data is collected, and the provider processes the data on behalf of the controller in accordance with the GDPR. Health data requires special protective measures and clear deletion periods.

What does the assistant do in an emergency?

It recognizes predefined emergency keywords, refers to emergency number 112, and immediately forwards the call to the on-call service. It does not provide medical assessments.

Can older callers handle an AI phone assistant?

A naturally speaking assistant is easier for many people than a keypad menu. Important factors are a calm speaking pace, patience with follow-up questions, and the option to switch to a human at any time.

Can the assistant change deployment times?

Only if this has been agreed with your team and the route planning system is connected. In many services, it first records change requests as a message, which management then confirms.
